It's like say two siblings that have the same parents are cousins and the siblings' actual cousins aren't related to them at all. Indra and Asura are brothers, they both had children to make up the Uchiha clan and the Senju clan also down the line Uzumaki clan.
So whatever children that they have are cousins. We all know the Indra and Asura's father is Hagoromo and we also know that Hagoromo had a brother named Hamura. So that makes the descendants of these four clans, Tsunade, Naruto, Sasuke and Hinata, distant relatives. We all must know that Tsunade and Naruto are related.
